If you are interested in joining our community of friends and family, please browse and complete the above forms and submit them to the Parish Office, either by emailing it to stmarydepere@itol.com, by mailing it to St. Mary Catholic Community PO Box 70 De Pere, WI 54115, or by bringing it in person into the Parish Office.
Brief History – gives a brief background of the Church (informational)
Census form – gives basic information about the person(s) attending the parish.
Time and Talent information sheet – gives information about various opportunities to be involved in the Parish.
Financial Commitment form – gives the Parish an idea of how much the new member(s) is able to contribute to the general operating/offertory fund that supports the functioning of the Church.
