Forms for Religious Formation

Four forms are required - Registration Form; Registration Fee Form; eNews Sign Up; and Permission and Insurance Release Form.

The Volunteer Form is optional. Be aware you can trade tuition for your children's classes for volunteering to teach classes, plus we will pay you. We will train you, if you want to teach.

Please submit your forms online. If you don't have a computer, you can use a computer at church. Only the Registration Fee Form must be physically sent in with payment. When you fill out forms online, we will automatically send you an email copy of the completed form for your records to the email address you enter in your form. Thank you!
